SPECIFIC INSTRUCTIONS:

Part I - General Information

  1. Fill in all details carefully and completely.
  2. Note that signature of the PI and Authorized Official of the institution attest to the fact that all information contained in the proposal is accurate.
  3. With regard to Part I.9 (Other Support and Financial Resources for Research), please note the following:

The information requested in this section is required for two purposes:

a) To insure that there is no direct overlap in research funding i.e. that research funded from other competitive sources is not funded concurrently by the Center.

b) To establish that the investigators have access to financial resources for research on Alzheimer's disease and related disorders which is at least equal to the sum requested from the Center. This is in compliance with the condition of matching funds established by the donor. The source of these financial resources may be one or more of the following: Competitive research grants which do not directly overlap with the project funded by the Center, institutional support in the form of stipends for students and/or direct non-competitive funding of research (but not salaries of investigators, laboratory technicians or other personnel nor equipment purchased by the institution), funds from donors, interest on endowments and other research funds, contracts with industry. Funding from these sources available to the PI and/or Co-Is (in any combination) will satisfy the matching requirement provided that it equals the amount requested from the Center to the research project.

Therefore, information is requested for the two categories of funding mentioned above i.e.:

a) Funding (existing or pending) from grants awarded on a competitive basis. For this category full disclosure is required i.e. all funded and pending grants must be reported by the PI and Co-PIs whether or not they are related to the proposed research project.

b) Funding from any other source. These funds need be reported only to the extent needed to fulfill the matching requirement.

Please provide the following information for the PI and each of the Co-PIs separately:

Competitive Grants Awarded (for each grant).
(a) Source/Agency (b) Title of project (c) Start date and end date of the project (d) Total amount of the award for the entire grant period (e) Amount for the current year (f) Degree of overlap with the proposed project - none, partial or complete. If the overlap is partial, indicate which sections overlap and which do not.

Competitive Grants Pending (submitted but not yet approved or approved but not yet funded; for each grant):
(a) Source/Agency (b) Title of project (c) Anticipated start date and end date of the project, if funded (d) Total sum of requested for the entire grant period (e) Amount requested for the first year (f) Degree of overlap with the proposed project - none, partial or complete. If the overlap is partial, indicate which sections overlap and which do not.

Other Sources of Funding for Research on Alzheimer's Disease and Related Disorders
e.g. Institutional support in the form of stipends for students and/or direct non-competitive funding of research (but not salaries of investigators, laboratory technicians or other personnel nor equipment purchased by the institution, funds from donors, interest on endowments and other research funds, contracts with industry. Only those sources of research income counted for matching purposes need be mentioned. For each item provide: (a) Source of Funding (b) Amount for the first year of the project.

Table of Matching Funds:
This should be filled in on the basis of the information given in the preceding section. The total must equal the total funds requested from the Center for the project.

NOTE: It is important that this section be accurately and comprehensively filled in.
